There are 2 reasons you would fill out this form.
- You would like to apply for a grant, and your previous grant Final Report will not be submitted before the grant application deadline. In this case you must submit this form well in advance of the Spring grant deadline. Decisions are made on a case by case basis. You will receive notification of whether your request is approved within 2 weeks of submission. Only if your request is approved, will WCF consider a new grant application.
- Your project cannot proceed as planned and you would like to request a change to the project. You may not make any significant changes to the project without submitting this form and receiving written approval. You may submit this request at any time during the project’s projected timeline.
